1. The parties to this agreement are (The Client) and EandOGlobal Insurance & Financial Services, Department of Insurance (Arizona License #3003108811 – California License #6004126) (“BROKER”).
2. (The Client) appoints BROKER as (The Client)’s insurance broker of record.
3. This agreement shall become operative on the (Effective date) and shall continue in full force until terminated by either party.
4. BROKER agrees to represent (The Client) honestly and competently.
5. (The Client) agrees to pay BROKER a broker fee for BROKER’S services. The broker fee varies. The broker fee IS NOT refundable.
6. BROKER may in the future charge (The Client), and (The Client) agrees to pay additional fee(s) for the services listed below. There may be additional fees for additional services.
7. Following are the nature and amount of all fees known to BROKER that will be charged by persons other than BROKER or the insurance company in connection with current placement of (The Client)’s insurance. These fees are not retained by BROKER.

By submitting information through this insurance quote form, I authorize E&O Global Insurance & Financial Services and its authorized insurance agents and representatives to collect, use, and process the information I provide for the purpose of evaluating my insurance needs, obtaining insurance quotations, and assisting me with the purchase, placement, renewal, or servicing of insurance coverage.

I understand that, in order to obtain insurance quotations or place coverage, E&O Global may submit the information I provide to insurance carriers, wholesalers, managing general agents (MGAs), brokers, underwriters, and other authorized insurance markets or service providers as reasonably necessary to evaluate risks, obtain quotations, determine eligibility, and place or service insurance coverage.

I understand that E&O Global may communicate with me regarding my request, including by email, telephone, or other contact information I provide, and may send me quotations, coverage information, applications, proposals, and related insurance documents.

I understand that the information I provide will be used for legitimate insurance-related business purposes and will not be sold by E&O Global as a consumer mailing list. Information may be shared with insurance markets and service providers when reasonably necessary to respond to my request, obtain insurance coverage, or provide related insurance services.

I understand that submitting this form does not constitute an application for coverage, a binder, or a guarantee that insurance coverage will be offered. Any coverage is subject to the applicable carrier’s underwriting requirements, terms, conditions, exclusions, and approval.

By using this website and/or submitting the insurance quote form, I confirm that I have read and understand this authorization and consent to E&O Global’s collection, use, and sharing of the information I provide for the insurance purposes described above.
